New Passport

Requirements:

If an applicant is in need of a brand new passport, a DS-11 is submitted. It applies to the following situations:

  • If your passport is damaged.
  • If you are applying for a passport for the first time.
  • If the person you are applying for is under 16 years of age.
  • If the last passport issued to you is equal to 15 years or more
  • If you were 15 years old or younger during the issuance of your previous passport.

Passport Renewal

Requirements:

If an applicant needs to renew their passport, a DS-82 is submitted. A prerequisite for this application is having the previous passport in your possession. You can use a DS82 if:

  • If you were 16 years old or older during the issuance of your previous passport.
  • If your previous passport is undamaged and under your possession.
  • If your previous passport has expired (within the last 5 years.) or is almost expired.
  • If you have undergone a legal name change and the issuance of your previous passport is equal to or greater than 1 year.

Lost or Stolen Passport

Requirements:

If an applicant needs to report their passport as lost/stolen then a DS-64 needs to be submitted. Use this if:

  • If a report needs to be made regarding a stolen or lost passport
  • If a report needs to be made regarding a stolen or lost passport and the applicant would like a new passport application.
